MAT Program Application Process

The Maryland Accessible Telecommunications (MAT) program provides assistive equipment to qualified applicants. To get started, please follow the application instructions below.

 1. Review the Qualification Criteria

    To qualify for equipment provided by the State, a MAT program participant must be:

  • A Maryland resident who has difficulty using a standard telephone
  • At least three years old (Applicants ages 18 and under must have an application signed by a parent or legal guardian)

  • Certified as being Deaf, hard of hearing, DeafBlind, blind, low vision or having cognitive, mobility or speech-related difficulties

  • On a fixed income, or a recipient of at least one of the following:

  • Social Security (SSA) benefits
  • Veterans (VA) benefits
  • Supplemental Security Income (SSI)
  • Social Security Disability Insurance (SSDI)
  • Temporary Disability Assistance Program (TDAP)
  • Temporary Assistance for Needy Families (TANF)
  • Pharmacy, medical or housing assistance

  • Equipped with home phone service, high speed/WiFi, or applying for phone service and/or high speed/WiFi. View our Special Programs page for additional information.


If you are not sure if you qualify, you may still apply. Free evaluations and other support may be available to non-qualified applicants, as determined on a case-by-case basis.

  2. Complete Application Online or Download and Complete the Application

  3. Get the Application Certified

Mail or take the application’s Disability Certification Form (labeled Part 4) to one of the following licensed professionals for certification:

  • Licensed physician
  • Audiologist
  • Speech pathologist​
  • Rehabilitation counselor


This person, acting within the scope of his or her license, will certify that you are unable to use a standard telephone, and that you can benefit from this program.

  4. Enclose the Proper Documentation

If you receive any of the following, please include a copy of your Social Security Administration Award Letter:

  • SSA
  • SSI
  • SSDI


    OR
Please send copy of paperwork as proof of eligibility:

  • Veterans (VA) benefits
  • Temporary Disability Assistance Program (TDAP)
  • Temporary Assistance for Needy Families (TANF)
  • Pharmacy, medical or housing assistance


    OR
    Document if your income is fixed or limited.

    Also required

  • Copy of photo ID
  • Copy of telephone or utility bill


  5. Read, Sign and Mail All Forms

  Mail the completed application and necessary documentation to:

Please note that if your form is not properly signed, or is incomplete in any way, it will delay your request for equipment. Applicants under the age of 18 must have their application signed by a parent or guardian.

After You Apply

Once your completed application is received, it will be reviewed by the MAT program for approval. If you are approved, you will receive an acceptance letter that includes information on scheduling your evaluation. The goal of the evaluation is to find the equipment options that meet your needs. Equipment will be mailed to you 4-6 weeks after your completed evaluation.
