What is the Attendant Care Program?
The Attendant Care Program provides financial reimbursement to assist individuals with severe chronic or permanent physical disabilities who require attendant care services to direct their own care and select their own service providers. Reimbursement may be available for attendant care services that are provided in the home, school, workplace, or other community locations.
Who Can Benefit from this Program?
Eligible adults age 18 to 64 with severe chronic or severe physical disabilities. Applicants must meet one of the following criteria:

Be self-employed or competitively employed with compensation;

Be actively seeking employment;

Be attending an institution of post secondary, higher education, or apprenticeship training program;

Be a resident of a nursing facility or similar institution who are seeking to transition into the community;

Be at risk of placement in a nursing home, as certified by the attending physician, if attendant care services are not received in the community.
How can I apply?
Maryland State has created a portal to house the state’s licenses, permits, applications, and registrations across numerous state agencies. The portal is called OneStop, and it is a resource for MDOD staff, supplemental administrators, and the public to manage their documents on a central hub.
Interested applicants must first register a OneStop account . In order to create your account, you will need to provide your first and last name as well as an email address you can verify.
Once your account is created, return to the home screen and select "Maryland Department of Disabilities". You must complete the short application in order for your name to be placed on the registry for the ACP program.
As space becomes available on the waiting list, you will receive a link by email to complete the full application.
